Eligibility Information
A staff member will contact you to set up the date/time to bring your child for screening. To help you be prepared for the screening, please bring copies of the following items to the screening: proof of income (current W2 or three months of paystubs for working members of the household), current immunization record, vision screening, dental screening, and physical/well child checkup.
A child may qualify for the preschool program if they: -- Are four years old by August 1 and meet income requirements, -- Have a recognized disability or speech delay at age four, or -- Have a disability or speech delay at age three, even if their fourth birthday is after August 1.
*COMPLETING THE APPLICATION AND SCREENING PROCESS DOES NOT GUARANTEE PLACEMENT IN THE PRESCHOOL PROGRAM.
Un miembro del personal se pondrá en contacto con usted para coordinar la fecha y la hora en que deberá traer a su hijo(a) para la evaluación. Para ayudarle a prepararse para la evaluación, por favor traiga copias de los siguientes documentos: comprobante de ingresos (formulario W-2 actual o los recibos de pago de los últimos tres meses de los miembros del hogar que trabajen), registro de vacunación actualizado, evaluación de la vista, evaluación dental y examen físico o de salud infantil.
Un niño puede ser elegible para el programa preescolar si: -- Tiene cuatro años cumplidos antes del 1 de agosto y cumple con los requisitos de ingresos, -- Presenta una discapacidad o retraso en el habla reconocido a los cuatro años, o -- Presenta una discapacidad o retraso en el habla a los tres años, incluso si cumple cuatro años después del 1 de agosto.
